• Students Birth Certificate

• Immunization Record

• Physical Records

• Parent Identification Card

If your child is religiously exempted from immunizations, please bring the supporting medical
documents.

If your child suffers from a specific food or environmental allergies, please bring the supporting
medical documents.

Non-refundable Fees due at time of enrollment:
Registration: $130.00
Supply Fee: Due in August/September and varies on the age of the child.
